Hospice RN Case Manager, you will:
- Provide patient care across the continuum, managing hospice
services with empathy and expertise.
- Assess, plan, implement, and evaluate individualized care plans
for patients.
- Support patients and families through end-of-life care with
compassion and dignity.
- Collaborate with interdisciplinary teams to ensure seamless
care delivery.
- Maintain accurate documentation and manage multiple priorities
effectively.What You Bring
- Education: Graduate of an accredited Certificate, Diploma,
Associate, or Baccalaureate Nursing Program.
- Licensure: Current Registered Nurse (RN) license in [Insert
State].
- Experience:
- Minimum of 1 year in a clinical care setting.
- Hospice or home health experience (preferred).
- Skills: Strong nursing skills, ability to manage priorities,
and dedication to patient-centered care.
- Transportation: Reliable transportation to perform job
responsibilities.
- Ability to sit, stand, bend, lift, and move intermittently,
