Care Management South Central contributes by providing clinical and
managerial oversight of Care Management programs and contracts.
This role requires a strong understanding of Ryan White Care
Management, program guidelines, HAB measures, standards of care,
and resources. You will work with regional teams to ensure
operational understanding, deliverables, and performance goals are
met. You will provide clinical guidance, monitor productivity,
analyze reports, and conduct 1:1 meetings to address challenges
collaboratively.You will also conduct internal audits, ensure
compliance, report on program successes and challenges, and serve
as a professional resource for HCCs. The role involves creative
collaboration to meet contract expectations, uphold the Ryan White
Care Act principles, and facilitate quality improvement
initiatives. Other programs or regions may be assigned as
needed.Success in this role depends on embodying AHF's core values
and mission, maintaining a nimble and adaptable approach, and
fostering collaboration across divisions to ensure an optimal
client experience. You will oversee local supervisors, troubleshoot
issues, act as a liaison, monitor progress, ensure policy fidelity,
and prepare reports to meet contract requirements.Licenses and
Certifications: Current and good standing Licensed Clinical Social
Worker or Registered Nurse from an accredited institution. Masters
preferred, with a minimum of 5 years experience in Ryan White
Program services and 2 years of management experience. Ability to
